The First US Tinnitus Guideline Update Since 2014 Names Bimodal Neuromodulation, Handing Neuromod’s Lenire a Coverage Argument

Neuromod Devices said on 16 October that the American Academy of Otolaryngology-Head and Neck Surgery Foundation’s updated tinnitus guideline includes bimodal neuromodulation, the approach behind its Lenire device, as a treatment option. The academy published the update on 12 October in Otolaryngology-Head and Neck Surgery and lists bimodal neuromodulation among its new key action statements, alongside self-harm screening, somatosensory tinnitus, cochlear implantation and follow-up. It recommends against routine use of medicines, dietary supplements and repetitive transcranial magnetic stimulation. Lenire pairs sound with mild electrical stimulation of the tongue, is prescribed by a clinician and used at home, and received FDA De Novo authorisation in March 2023.

A guideline listing is the first thing a payer asks a device maker for. Lenire now has it. The same update steers clinicians away from pills, supplements and rTMS, which leaves fewer alternatives for a bothered patient once counselling and sound therapy are in place.

  • Neuromod: the grade of the statement decides how far it travels. The company describes the listing as a treatment option. If the guideline grades it as an option, it supports case-by-case approvals rather than routine coverage, so the next step is converting the citation into insurer medical policies.
  • The VA channel: the largest tinnitus population already has a route. Tinnitus is the most common service-connected disability among US veterans, and Lenire is available through the Department of Veterans Affairs. A guideline citation gives VA audiologists and ENTs a documented basis for prescribing it.
  • Signposts: medical policies, Medicare and the next device. Watch for commercial insurers updating tinnitus policies to cite the guideline, for any Medicare coverage of the device category, and for rival neuromodulation devices seeking the same statement in the next update.
