AbbVie’s Skyrizi Combination Nearly Doubled Endoscopic Remission at 24 Weeks in Crohn’s, in an Open-Label Study Without Placebo

AbbVie said on 19 October that ABBV-466, which combines its IL-23 inhibitor Skyrizi (risankizumab) with trosunilimab, an investigational α4β7 inhibitor, achieved higher endoscopic remission than either drug alone in the Phase 2a TARGET-CD study in moderately to severely active Crohn’s disease, presented as a late-breaker at UEG Week 2026 in Barcelona. The open-label platform study randomised 300 patients equally to 24 weeks of the combination, risankizumab or trosunilimab. Three in four had failed at least one advanced therapy, and a quarter had failed three or more. The study is continuing.

30.2%
Endoscopic remission at week 12 with ABBV-466, against 21.3% with risankizumab and 18.0% with trosunilimab
40.5%
Endoscopic remission at week 24, against 22.4% and 20.0%
10.4%
Serious adverse events with the combination, against 9.6% and 9.0%

Clinical read

The gap widened between week 12 and week 24, and that is the stronger signal. The combination’s lead over risankizumab grew from about 9 points to 18 points of endoscopic remission while both single agents levelled off. With no placebo arm and no statistical testing reported in the release, the trajectory says more than any single figure.

  • Prescribers: this was tested in the hard-to-treat patient. Most patients had already failed an advanced therapy, including some who had failed an IL-23 inhibitor or a JAK inhibitor. If the effect holds in a controlled trial, the first use is in refractory disease, where options are thinnest.
  • Takeda: the α4β7 mechanism behind Entyvio now has an in-house AbbVie rival. Trosunilimab gives AbbVie its own gut-selective α4β7 inhibitor to pair with Skyrizi. A fixed combination would let AbbVie extend its IL-23 franchise and compete on Entyvio’s mechanism within one product.
  • Signposts: the full data, the next trial and infections. Watch for doses and clinical remission in the full dataset, the design of the next randomised study, and infection rates as exposure grows, the main risk when two immune-targeted biologics are combined.
