J&J Built Its Surgical Robot Into the Table, and Aimed It at the 92% of Surgery That Isn’t Robotic

  • The FDA granted De Novo authorization to Johnson & Johnson’s OTTAVA Robotic Surgical System, which the company calls the world’s first table-integrated soft tissue robotic system. J&J submitted the application on 7 January 2026, so the review took roughly six months.
  • Authorized for ten general surgery procedures in the upper abdomen, including Roux-en-Y gastric bypass, gastrectomy, cholecystectomy, gastric sleeve, appendectomy and hiatal hernia repair.
  • Four robotic arms are built into the operating table rather than mounted on booms or carts, cutting the system footprint by 30 to 50 percent and, J&J says, letting it work in theatres that could not previously fit a robot.
  • J&J will start a US commercial launch with selected customers and is planning launches in Japan and Western Europe. An inguinal hernia trial is expected to support a second US submission early next year.
CI read The footprint is the strategy, and it is aimed somewhere other than the obvious target. Global robotic penetration of surgical procedures is about 8 percent, and the binding constraint has never been surgeon appetite, it is that most operating rooms physically cannot accommodate a boom-and-cart robot. By building the arms into the table, J&J is going after the 92 percent of procedures that are not robotic rather than fighting Intuitive for the 8 percent that are. That is an expansion play dressed as a challenger play.
