France Trades €100 Million in Price Relief for an Annual Review of Older Drugs
France’s government presented its 2027 social security financing bill (PLFSS) on 1 October, pairing a smaller drug price-cut envelope with a tighter cap on health insurance spending growth (ONDAM). The government presents the lighter price cuts as support for the arrival of particularly effective new medicines. Alongside that, the bill introduces what the government calls a golden rule on reimbursement: an annual review in which the Haute Autorité de Santé (HAS) can give an opinion on whether certain medicines should stay reimbursed as more effective treatments arrive. Parliamentary examination begins in October.
France has moved the reimbursement threat from price to presence. A smaller price-cut envelope eases the annual squeeze, but an established brand’s place on the reimbursement list can now be questioned once a better treatment launches. For incumbents, a competitor’s arrival in France becomes a reimbursement event as well as a commercial one.
- Established brands: the review trigger now sits with competitors. HAS will be able to give an opinion on keeping a medicine reimbursed in light of newer, more effective options. Brands with a stronger entrant on the horizon should have comparative evidence and a price position ready before the review opens.

The rule ties a newcomer’s arrival to a review of what it replaces, and the government links the smaller price cuts to supporting particularly effective new medicines. A dossier that shows superiority over a named incumbent now works twice, for the entrant’s price and against the incumbent’s listing.

The review criteria, the timetable and any route to contest an HAS opinion will decide whether this reaches a handful of products or the whole list. With ONDAM growth capped at 2.0% and the deficit target cut from €21.8 billion to €12.7 billion, any amendment that softens the rule will need savings found elsewhere.
