Jaypirca’s 80% Win Over Chemo Opens a First-Line CLL Fight With Three BTK Rivals

Eli Lilly announced on 2 October that FDA approved Jaypirca (pirtobrutinib), the only approved non-covalent BTK inhibitor, for adults with previously untreated CLL or SLL with no known 17p deletion. The approval rests on BRUIN CLL-313, in which 282 patients were randomised to pirtobrutinib or bendamustine plus rituximab; at a median follow-up of 28 months, median progression-free survival was not reached against 33.5 months for chemoimmunotherapy. Jaypirca has held a US approval in relapsed CLL after a covalent BTK inhibitor since December 2025.

Lilly has taken its post-covalent rescue drug into the first-line fight with ibrutinib, acalabrutinib and zanubrutinib, on a win over chemoimmunotherapy rather than over any of them. With bendamustine plus rituximab as the comparator, the first-line case will run on tolerability and on BRUIN CLL-314, the head-to-head against ibrutinib that Lilly describes as positive.

  • Covalent BTK incumbents: the attack line is the cardiac patient. NCCN recommends pirtobrutinib first line for older patients with cardiac comorbidities, and atrial fibrillation or flutter ran 1.4% in CLL-313. The rebuttal sits in the same release: the trial excluded significant cardiovascular disease, and Jaypirca’s label carries an arrhythmia warning.
  • Lilly’s own franchise: first-line use spends the second-line card. Jaypirca’s established role is rescue after a covalent BTK inhibitor, where NCCN lists it Category 1 preferred. Lilly’s release notes there are no data on sequencing after progression on pirtobrutinib or on cross-resistance to covalent agents, so moving it earlier puts the next step into unmapped territory.
  • Signposts: the full CLL-314 numbers and first-line del(17p). Head-to-head data against ibrutinib will decide formulary position more than this label. Del(17p), excluded here and present in 5% to 8% of patients at diagnosis, stays outside Jaypirca’s first-line indication for now.
