Korea Now Reimburses BeOne’s Tevimbra First-Line in Three Cancers, Using PD-L1 Cutoffs to Set the Limits
- Korea’s national health insurance now covers Tevimbra (tislelizumab), the PD-1 inhibitor from BeOne Medicines, in five indications across oesophageal, gastric and lung cancer, BeOne Medicines Korea said on 1 October 2026. Coverage was previously limited to second-line oesophageal squamous cell carcinoma, reimbursed since April 2025.
- The new indications are first-line oesophageal squamous cell carcinoma with PD-L1 of 10% or more; first-line HER2-negative gastric or gastroesophageal junction cancer with PD-L1 of 5% or more; first-line squamous NSCLC; first-line non-squamous NSCLC with PD-L1 of 50% or more; and NSCLC after platinum chemotherapy.
- In first-line oesophageal cancer, RATIONALE-306 showed median overall survival of 17.2 against 10.6 months (HR 0.66). In first-line gastric cancer, RATIONALE-305 showed 15.0 against 12.9 months (HR 0.80).
- First-line NSCLC coverage includes stage IIIB to IIIC disease ineligible for surgery or radiotherapy, where BeOne says other immune checkpoint inhibitors are not reimbursed first-line in Korea.

One reimbursement decision moved a second-line PD-1 into first line across three of Korea’s common cancers, inside biomarker limits.
- In Korea, coverage is the market. Going from one reimbursed indication to five changes Tevimbra’s addressable population far more than any approval would, because use follows national insurance coverage.
- The PD-L1 cutoffs are the price of breadth. Three of the five indications carry thresholds of 10%, 5% or 50%, narrowing who qualifies. The survival benefit also differs by tumour: a hazard ratio of 0.66 in oesophageal cancer against 0.80 in gastric cancer.
- A price contest is coming. Korea Biomedical Review reports that Opdivo is moving closer to Korean coverage in oesophageal and liver cancers, setting up a price battle. BeOne’s claim of a first-line stage III NSCLC position that other checkpoint inhibitors lack is the kind of reimbursed niche that matters when prices start to compete.
